Billing & Insurance
As a courtesy to our patients, we will file the necessary claim forms to all insurance companies even if we are not a participating provider. Please remember that we cannot guarantee payment by your insurance company.

- Medicare and all Medicare replacement policies

Co-pays and co-insurance payments are due at the time of service. Any remaining amount not covered by insurance is due upon receipt of the statement from Eastern Virginia Ear, Nose & Throat Specialists. It is important that you familiarize yourself with the exclusions and limitations of your insurance contract.
Insurance Cards and valid photo ID are required at the time of service.
If your insurance requires a referral, the patient is responsible to bring a copy of the referral or to confirm in advance that the referral has been received prior to the appointment.
We accept AMEX, VISA, MasterCard and Discover. Discounts offered for patients without insurance. Call the Billing Office for a quote.
